NEW FOR 2007 - ATTENTION ENTRANTS – PLEASE USE OUR INTERACTIVE ENTRY FORMS
We highly recommend all Entrants use our PDF “Interactive Team Entry Form”, “Banquet Reservation Form”, and “RV Reservation Form” for ease of data entry and legibility (and the Trial Secretary / Banquet Chair / RV Chair will truly appreciate your consideration). Our Interactive forms are available as separate download files located with the DOCOF Doings on our web site. You may enter your data, edit, save, and print the completed form all as a “PDF” file using your computer. The saved “Team Entry Form” may be sent as an email attachment for proof reading and sharing with individual Team members. The form is user friendly with the same format as our manual Forms, but contains text fields and drop down menus for ease of computer entry. Most text fields are dynamic and adjust the type size to fit the amount of data entered. Using the keyboard Tab key moves sequentially through the form fields. Clicking outside the Form stops your data input. Click a field to restart or edit data. Clicking the ‘Highlight Fields’ icon in the Message Bar will toggle the highlighting On and Off in the form fields. No special software is necessary but you will need Adobe Reader, Version 7.0.5 or greater*, dependent on your computer operating system. As DOCOF requires the Team Entry Form, Banquet, and RV Reservation Fees to accompany your entry, you must print and sign the form, enclose your check, and mail to the Trial Secretary, Banquet Chair, or RV Chair listed on the Form. Email entries are not valid for these Fee related Forms. You will be able to begin entering your data immediately, as the Forms are very intuitive. However, you can click on Adobe Help in Reader and Search “Forms” for additional information. Please email the Trial Secretary or Banquet Chair or RV Chair with specific questions. * Please use this Adobe Reader link < Adobe Upgrade > to ensure you are using the latest Reader version for your Operating System. Older versions may not support all features of this Interactive format, so it’s a good time to ‘get current’. The Adobe Reader download is FREE. From the table below select "Interactive" to download the interactive PDF form; select "MS Word" to down in MS Word format; or select "PDF" to download in regular PDF form which you can print and fill out by hand.
